CarePortal Church Enrollment

This form is for church leaders that are setting up their CarePortal account for the first time. If you have any questions please contact us here.

Step 1: Enroll Your Church

Step 2: Verify Your Email (through email provided at enrollment)

Step 3: Access Your Dashboard  at https://system.careportal.org/login.  Or at www.careportal.org

Step 4: Complete church demographic information including church address and affiliations.  Also confirm CarePortal Statement of Faith, Terms of Service, and Who We Serve information.  This clarifies that you are a Christian Church, understand how CarePortal Works, and are willing to serve all people when and if you respond to a request.

Step 5: Access training at – www.111project.org/church-orientation

Step 6: Meet with your 111Project local staff person
